What is RNY Gastric Bypass Surgery?
n RNY surgery, the stomach is completely divided into two compartments —an upper part, or pouch, and the lower portion of the stomach.
Where to cut ?
The upper pouch is smaller, and is made to hold one ounce (about the size of an egg) or less. The small bowel is divided and brought up to the upper stomach where an anastomosis is made between the pouch and the small bowel. An anastomosis is the term surgeons use for the opening formed when they sew, or staple together, two pieces of bowel, making an opening—or stoma—between them. The Y-connection allows pancreatic fluid and bile to aid in absorption of nutrients
The opening between the new pouch and the intestines is called a stoma.
The stoma size keeps food in the pouch, allowing the patient to feel full for several hours on a small amount of food.

What is Pcos?
Overview
Polycystic ovary disease is a condition in which there are many small cysts in the ovaries, which can affect a woman's ability to get pregnant.
Polycystic ovary disease is a condition in which there are many small cysts in the ovaries, which can affect a woman's ability to get pregnant.
Symptoms
If you have polycystic ovary disease, you are likely to have some of the following symptoms:
~Abnormal, irregular, or very light or infrequent menstrual periods
~Absent periods, usually (but not always) after having one or more normal menstrual periods during puberty (secondary amenorrhea)
~Acne that gets worse
~Decreased breast size
~Development of male sex characteristics (virilization), such as increased body hair, facial hair, a deepening of the voice, male-pattern baldness, and enlargement of the clitoris
~Diabetes
~Increased hair growth; body hair may be in a male pattern
~Infertility
~Poor response to the hormone, insulin (insulin resistance), leading to a build-up of insulin in the blood
~Weight gain, or obesity
Treatment
Medications used to treat the symptoms of polycystic ovary disease include:
Birth control pills
Clomiphene citrate
Flutamide
Spironolactone
Treatment with clomiphene citrate causes the pituitary gland to produce more FSH. This causes the egg to mature and be released. Sometimes women need stronger fertility drugs to get pregnant.
In women with polycystic ovary disease who also have insulin resistance, glucophage (Metformin), a medication that makes cells more sensitive to insulin, has been shown to make ovulation normal.
Losing weight (which can be difficult) may help to reduce the high insulin levels in the blood. For women with this condition who are overweight, weight loss can reduce insulin resistance, stimulate ovulation, and improve fertility rates.
Causes
Polycystic ovary disease affects hormone cycles. Hormones help regulate the normal development of eggs in the ovaries. It is not completely understood why or how hormone cycles are interrupted, although there are several ideas.
Follicles are sacs within the ovaries that contain eggs. In polycystic ovary disease, there are many poorly developed follicles in the ovaries. The eggs in these follicles do not mature and, therefore, cannot be released from the ovaries. Instead, they form cysts in the ovary.
This can contribute to infertility. The immature follicles and the inability to release an egg (ovulate) are likely caused by low levels of follicle stimulating hormone (FSH), and higher than normal levels of male hormones (androgens) produced in the ovary.
Women are usually diagnosed when in their 20s or 30s. Women with this disorder often have a mother or sister who has symptoms similar to polycystic ovary disease.
Tests & diagnosis
During a pelvic examination, the health care provider may note an enlarged clitoris (very rare finding) and enlarged ovaries.
Tests include:
Abdominal ultrasound
Abdominal MRI
Biopsy of the ovary
Estrogen levels
Fasting glucose and insulin levels
FSH levels
Laparoscopy
LH levels
Male hormone (testosterone) levels
Urine 17-ketosteroids
Vaginal ultrasound
Blood tests that may be done include:
Pregnancy test (serum HCG)
Prolactin levels
Thyroid function tests
Prognosis
Women who have this condition can get pregnant with the right surgical or medical treatments. Pregnancies are usually normal.
Complications
Increased risk of endometrial cancer
Infertility
Obesity-related conditions, such as high blood pressure and diabetes
Possible increased risk of breast cancer

I will be going threw a major changing life surgery January 29Th 2010. I am getting RNY Gastric Bypass Surgery. Dr James M.Swain is my surgeon and i will be at St.Marys in Rochester..
Lets start.............
My Name is Teresa and I'm 24 years old. I'm happily Married to my husband of 5 years.. We got married June 19th,2004. It was a beautiful wedding!! We went to Duluth for a week for our Honeymoon. It was amazing and so relaxing... After we got married we wanted to start a family...we tried for 4 years and nothing..besides big fat negatives..it was so sad! I knew it was going to be hard because i had "pcos"...I was diagnosed with PCOS also know as Polycystic Ovary Syndrome in the 7th grade... Pcos sucks...and it sucks when you go threw high school and people make fun of you...only if they new truly what you were going threw...
Well Pcos makes you gain weight..and boy is that true...didn't gain it anywhere else besides my belly. My legs, arms, or face never got to big..but you could tell all my weight was in my stomach. So it all begins.................
Leo and I wanted to get pregnant more then anything, our friends, and family members everyone was pregnant... and we wanted to be so bad...
We went to our doctor and talked with her so she decided to put me on Metformin it helps woman who have pcos and are trying to loose weigh..it is also a diabetic medication.. well i was on metformin for well over a year..and it did NOTHING for me... So back to square 1...agian...
We went to a fertility specialist at Olmsted in Rochester. They did a million blood test, and ultrasounds and everything came out great nothing wrong.. besides i don't ovulate..big problem when your trying to get pregnant.. no period no ovulation no pregnancy... So She told me to exercise get some weight off and stay on metformin... so i did that...got a few lbs off nothing to major.. still no period, still no ovulation.. i think i went threw a million ovulation sticks... ALL NEGATIVE........all i wanted to do was CRY, CRY, CRY............well i didn't give up... So back to omc They put me on a pill to give me my period "prometrium" So ya...got my period...yippie, however still no ovulation... 3 months of that...and no ovulation..at this time i was afraid nothing was going to work.. i was about to just give up... my thoughts were "i'm never going to be a mom" "I'm broken" I'm not a good wife because i can't make him a dad" Well back to omc.........ya........Now They put me on prometrium, and Clomid, and Metformin.. I read and heard so much good news about pcos women getting pregnant on there 1st or 2nd try of Clomid..so i was praying, and hoping, and praying.......... So Month one........Nothing...a period but No Ovulation........Month 2...Nothing...Month 3......Nothing...So i was clueless...where do we go from there? Well My OBGYN...had a ton more test ran, and then sent me over to Mayo...Oh Great i was thinking more doctors, more bad news, more testing..ugh............was not looking forward to it..However.. I did end up with an amazing Fertility Doctor who was all about me and my hopes and dreams... But my dreams were crushed the day she told me there was nothing else they could do since i don't ovulate... she said.. Your best bet for you would be Weight Loss Surgery... i was like......aww Hell No.. that's not for me.. i don't want to do that, I'm a 100% against it.. Well being so stubborn i didn't want to hear anymore of that..but she did want me to go talk to a weight loss Surgeon.. so i did.........and after talking to him.. my mind changed.....i was only about 50% against it.... So i need more convincing.. so i did research, talked to an OBGYN who specialises in Weight Loss Surgery Pregnancy's..and he was amazing.. i think after talking to him.. i was down to only like 10% of no surgery............. But was ready and willing to go for it..learn more..and make this big step in my life...
I went to Weight Loss Surgery Information Class, with my Mom and Mother-In-Law
That was the day my life changed forever, i new i wanted it.. i new i wanted to be healthy and happy!
The next day i called and made and apt to talk to a surgeon..and we started from there...

I can tell you was not Happy about joining the 12 week weight loss program, i truely thought it was going to be stupid and a wast of my drive from home to roch and it was a wast of gas... Well...it wasn't...it was one of the best things i did!! I have made so many friends and got so much information and had such an amazing teacher. In the 12 weeks i took that class i lost 15lbs..yeppers 15lbs on my own...Yippie!!
After my class was over, i had to meet with the surgeon for one last time before we gave the insurance company my information.. that appointment went GREAT......They gave my insurance company all my paper work and i received a approval letter in 1 1/2 weeks.. that was the best day ever.......... 4 days later the Mayo Clinic nurse called and we set up my surgery date.. January 29th is my surgery date.. I'm ready
